Clinical Care Coordinator

About the Position:

This position, also known as the Clinical Team Lead, coordinates all aspects of behavioral health treatment for approximately 7-10 foster youth. Youth present in our program with a range of behavioral and mental health needs.  While in the program, youth live with specially trained therapeutic foster parents and receive high levels of behavioral health care. The Team Lead acts as the primary clinician to coordinate the youth’s clinical team, which consists of a skills trainer, individual therapist, family therapist, and the therapeutic foster parent. The Team Lead provides direct support and coaching to the foster parent to ensure that interventions in the home align with the youth’s needs and model fidelity.  We address the needs of the whole child and work with a variety of partners to ensure long-term stability.

Salary Package comprised of:

  • Starting Salary: $69,500- $77, 875 per year depending on education, experience, and/or bilingual capabilities.
  • Bonus structure in place for on-call duties and caseload/billing incentive.  
  • 6-month review: performance-based increase up to 3%
  • Excellent benefits package
Job Duties:
  • Lead a team of individual and family counselors and skills trainers. 
  • Oversee all aspects of each child’s clinical program placement, making clinical decisions that align with model fidelity while also considering adaptations where needed, to align with youth’s identities and unique needs. 
  • Provide essential daily support to OCP’s therapeutic foster parents. Position includes on-call responsibilities by phone, with occasional in-person response outside of regular business hours. On call rotation possibilities available.  
  • Coordinate with schools, probation departments, mental health agencies, child welfare, CCO’s, culturally specific programs and other community partners. 
  • Facilitate weekly meetings including those with foster parents and respective clinical team. 
  • Ensure compliance with all applicable state, county and other regulations, record-keeping and case documentation. 
Job Requirements:  
  • Must qualify as a QMHP in Oregon.  Graduates from master’s programs in the fields of Social Work, Family Therapy, Counseling, Behavioral Science or related field are encouraged to apply. 
  • Proficient in MS Office. Comfortable learning Electronic Health Records, and other agency web-based platforms. 
  • Comfortable with discussing personal and professional growth including areas of cultural humility 
  • Valid driver’s license and access to personal vehicle.
