Appointment Scheduling and Missed Appointments:
The initial appointment is scheduled for 90 minutes, follow up appointments are scheduled for 30 minutes. OCP will
call to schedule a follow-up appointment according to Dr. Pareek’s suggested return to clinic date.

• If you need to reschedule or cancel an appointment, please call or email the psychiatry assistant.
• If you are running late for an appointment, please call the OCP receptionist at 541-743-4340.
• If you arrive more than 10 minutes past your appointment time, you will need to be rescheduled.
• Late Arrivals & Cancellations with fewer than 24 hours’ notice will be considered a no-show.

We will communicate with you following a missed appointment. Please note, two or more no show or late cancellations may result in your child being discharged from the medical management program at OCP.

Medications and Medication Refills:
For medication refills please call or email the psychiatry assistant.

Requests received Wednesday through Friday may not be filled until the following week.

Please call for a refill when you have AT LEAST seven days’ worth of medication to ensure there is no lapse in medication
dosages.

Parent or Guardian Participation:
A parent or guardian must attend every appointment. If your schedule changes and you cannot attend, please call
the psychiatry assistant to reschedule.
