Upon receipt of complete referral packet, OCP staff screen and respond within 5 business days about enrollment status.
QMHP providers wishing to refer a youth should work in collaboration with the family and submit this completed referral form AND the following documentation to intakecoordinator@oregoncp.org
Required:
- ROI signed by the legal guardian permitting exchange of info between referral source and OCP
If applicable:
- Most recent psychiatric evaluation
- Psychological testing results
- Behavioral Health treatment records from the last 60 days
- Most recent CANS assessment
