Dear Parent or Caregiver,


Greetings! Oregon Community Programs is committed to providing the best possible care to you and your family. We have chosen to partner with Boise State University (BSU) on a research project to help understand how to make youth mental health services even more effective.


BSU would like to invite you to participate in this research study. The goal of the study is to understand how therapists use assessment tools in their work with families and how to best support therapists and families in this effort. If you agree to participate, you will be asked to complete a short (8-10 minute) web based survey this week and once per month for 6 months. Surveys will ask about your child’s well-being and your experience with services here at Oregon Community Programs.


Each time you complete a survey, you will receive a $15 Amazon gift card. By learning about your experiences, we can get a better idea of how to best support therapists and families to use effective new tools.


It is important that you know that you and your child’s ability to receive services here at Oregon Community Programs is not affected in any way by your decision to participate or not participate in this study. Also, by returning this form you are not agreeing to participate, you are only agreeing to have a member of our research team contact you to share a bit of information about the study. The contact information you provide will only be used for this study. All information you share with the research team will be strictly confidential and will not be disclosed to Oregon Community Programs or anyone else.
